The Escharotic Process

This is a rather vivid example of the escharotic action. The patient had a very large breast tumor. Upon application, the paste first produces a heat reaction and reddening of the skin. This is usually followed by blistering, generally greenish-yellow in color. Then, an eschar begins to form. It gradually thickens and darkens. After approximately 12-16 days, sometimes longer, sometimes shorter, the eschar starts to crack around the edge and begins to detach. After the eschar detaches, site inspection becomes essential. It is necessary to determine whether there is any malignancy left in the site. The site is usually poulticed and it tends to discharge a great deal of morbid material for some days to several weeks or months.

Eventually the area heals. Depending on the exact nature of the original problem and the type of salve used, the tissue may be discolored for some time, usually not permanently. This woman had a mass about 3 cm. in diameter. The hole in her breast was at one time an inch and a half deep, perhaps even two inches. There was profuse discharging for several months and then healing such as is seen above.
